Plot
Dr. Maddox, Kelso's replacement, makes a few changes around the hospital. Meanwhile, the new incompetent interns have J.D. frustrated, and Elliot tries to get a handle of her ego.

Trama
Nella prima puntata dell'ottava serie fa il suo ingresso Courtney Cox come nuova primario dell'ospedale con il nome di Dr. Taylor Maddox. J.D. sarà alle prese con nuovi specializzandi che lo faranno andare fuori di testa, fino al punto di rifiutarsi di continuare a occuparsi di loro. Katie è un'arrampicatrice che fa di tutto per risaltare più degli altri, Denise è insensibile verso i pazienti ed Ed è interessato a tutto fuorchè alla medicina e ai pazienti. Il Dr. Cox è convinto che il nuovo primario sia un'idiota, ma si rifiuta di parlarci. A Elliot viene fatto notare invece che è un egoista egocentrica, soprattutto per il modo con cui tratta Keith. Alla fine, J.D. verrà convinto da Cox a riprendere i suoi specializzandi, perchè è loro dovere anche insegnare ai nuovi medici. Cox parlerà con Maddox, che si rivelerà essere essattamente quello che lui temeva: un'idiota attaccata ai soldi. Elliot si renderà conto di essere stata davvero pessima con Keith e gli chiederà scusa. Il vero colpo di scena però è il licenziamento dell'Inserviente da parte di Maddox dopo uno dei soliti scherzi che lui ha fatto a J.D. Come sarà il nuovo inserviente?
